Certification Reimbursement Pre-approval Form

At MiTech, we strive to improve personal and organizational effectiveness for the benefit of increasing staff skills which directly benefits our partners. Therefore, we encourage our staff to improve their skills by reimbursing the cost of certification test fees*.

To request reimbursement for a certification, please complete this pre-approval form. If your request is denied, you will be informed of the reason of denial. Once you pass the test, please submit a Certification Reimbursement Request Form.

* Note that certification test fees are reimbursed if test is passed only.
